Deadlines for Certificated Evaluation – Provisional, Long Form, and Short Form

Due by October 5th

Professional Growth Plan for each certificated employee. 

Use form in Appendix #5A

Due by November 30th

1st Observation of Provisional Employees.  Must be 30 continuous minutes followed by written summary with in 3 work days.  (Original must be sent to Personnel)

Use form in Appendix #5B

Due by January 15th

1st Observation of continuing employees on “long form” evaluation cycle.  Must be 30 continuous minutes followed by written summary with in 3 work days.  (Original must be sent to Personnel).

Due by February 25th

2nd Observation of Provisional Employees. Must be 30 continuous minutes followed by written summary with in 3 work days.  (Original must be sent to Personnel)

Due by March 1st

Year-end evaluation of Provisional Employees.  Employees evaluated as “unsatisfactory” must be placed on a plan of assistance with support to improve performance by May 1st. (Original must be sent to personnel)

Provisional Employee Evaluations:  New to Teaching or to WA State = 2 provisional years.  At least 2 years experience in WA State but new to District = 1 provisional year

Continuing Employee “Long Form” Evaluations:  For any teacher on a continuing contract, however teachers with 4 years of “satisfactory” long term evaluations qualify for the short form option below.

Due by October 5th

Continuing Employee “Short Form” Evaluations:  Option available to any continuing teacher with 4 years of “satisfactory” evaluations.  Can be on short form for 3 consecutive years, then must be on long form for at least 1 year.  Any teacher on a short form who is experiencing performance issues can be placed immediately on the long form by the principal.
